Tanzania Health Surveillance Form

Tanzania Health Surveillance Form – General information

Tanzania Health Surveillance Form is a mandatory health declaration for all incoming visitors to Tanzania. This document contains questions on symptoms of COVID-19 disease and other contagious illnesses.

The Tanzania health form was implemented by the Tanzanian authorities because of the COVID-19 pandemic to monitor the health of people willing to enter Tanzania. This way, travelers do not put at risk Tanzanian citizens and visitors, and minimize the spread of COVID-19.

The Tanzanian health form must be submitted within 24 hours before one intends to enter Tanzania. The application process is easy and takes place online.

A visitor needs to provide the necessary information, answer the questions, and cover the fees. The Tanzania Health Surveillance Form will be sent via email. Make a printout and bring it to the entry point, as the Tanzania health declaration will be requested upon your arrival.

Note: since May 1, 2022, and until further notice, the Tanzania Health Surveillance System has been suspended, as it is currently being maintained to enhance its capacity for recognizing and informing about infected travelers.
